Registration Information

1. Fill out the Registration Application, Registration Liability Form, and Health Form completely, by neatly printing all requested information.

 

2. A separate completed form is required for each camper.  If you need additional forms, contact the camp.

 

3. Please note that the form provides areas for both the parent and child to sign.  Maple Grove will treat any personal information about your child/applicant with great discretion, informing only those persons who need to know. (for example, but not limited to, the camp nurse or your child’s/applicant’s counsellor) please inform us if your child/applicant is on medication, has a history of emotional or behavioural problems, was recently hospitalized, or responds best to certain kinds of treatment.

 

4. Total payment is to be included with registration.

 

5. Early bird price/discount only applies until June 1

 

6. Late registration will be accepted throughout the summer provided payment in full is received with the application and subject to space availability.

 

7. Multi-camp discount only applies to families sending more than one child to camp.  Families registering multiple campers are eligible to receive a $25.00 discount off each additional camper rate.  All registrations must be received in the same envelope.  All campers must reside in the same household. 

 

8. If you need assistance in providing the funds for your child to attend Maple Grove, please contact us either by phone or e-mail.

 

9. A $25.00 charge will be applied to an NSF cheque.

 

10. Cancellations made in writing three weeks prior to the camp session will receive a refund of the registration fee less $50.00. No refunds will be issued if the cancellation is made less than three weeks before the camp session starting date.  No refunds will be issued due to late arrivals, early departures, home sickness, or dismissal due to disciplinary action.  Cancellations must be done in writing

 

11. After your registration has been processed, you will receive a confirmation letter. 

 

12. Please do not include tuck money in registration. A suggested amount of $20.00 can be brought with the camper.
